Sunderland Projects Update : Changing Lives September 2014

All projects focus on work with adults with multiple and complex needs with the focus of all but GAP being on homelessness as the main criteria.

 

Tatham House Project – We continue to support residents in Tatham House (alongside C4 and Turning Point). This approach is based on The Lodge model developed by St. Mungo’s charity in London. We are funded to engage with clients in the private hostel through 1-2-1 and group work, providing support to access services and look at move-on when appropriate. This project is funded until 31/12/14.

 

Norfolk Hotel – We run a non-commissioned weekly drop-in a the Norfolk Hotel, supporting residents to engage in services around health and wellbeing. Norfolk residents are also invited to Tatham House group activities both in-house and outdoor group activities.

 

Homeless / Supported Accommodation SP Contract (Assertive Outreach) – As part of the new Sunderland City Council Supported Accommodation contract, we are providing the assertive outreach element.

This includes working with rough sleepers, those who are sofa surfing, and those living in the private hostels who are at risk of homelessness. Our role is to try and secure temporary accommodation for clients and engage/re-engage clients with services across life domains. This team will respond to any sightings of rough sleeping activity.

 

GAP – We have secured funding for 1.5 GAP workers in Sunderland supporting women who are sex working and/or at risk of sexual exploitation. Support encompasses all emotional and practical assistance including safety planning.
